I'd guess Maine. The Acadia region is more than a little bit spooky.
Though don't sell Pennsylvania short. I don't know about per capita because the state has two major, largely outhouse-free population centers, but the middle of the state surely cranks up the gross outhouse total.
I've also been told there's some mostly feral folks out in the hollows of the Catskill region in New York who'd view an outhouse as an unnecessary modern amenity.

I'm not sure how this got to this point, but here's the census data on outhouses etc. I'll let one of the stat guys figure out which has the highest per capita, but one Google entry suggested it's Wisconsin.
