Has anyone ever heard of "Fourth Out" in baseball?

I have never heard of this before until I saw it mentioned on Alyssa Milano's Twitter page.

If you are the pitcher, you can strike-out the batter for the 3rd out, but if the catcher drops the ball and the runner reaches 1st base before the catcher can throw you out, your safe. So you have to get a 4th out. Was this what he was talking about?

Not sure, but when I hear "four outs" I think of the situations like we just saw in the first inning in tonight's game--giving the team an extra out by making a boneheaded mistake like not running from the mound to cover first.

Other observations from the beginning of tonight's game include:
1. Nice to see Bruce get a hold of one.
2. Also nice to see EE draw yet another walk.
3. More of the same from the bottom of the lineup as RH grounds into his 500th double play of the season. Cant wait to see what Gonzo's gonna produce in the 3rd...

Kind of confusing:

Try this breakdown:
1. Runners on Second and Third: 1 out
2. Liner back to the pitcher. 2 out. Runner at third runs and races home before the runner at second is doubled up. 3 out
3. Now the runner does score because it occured before the third out. However, the defense could have easily doubled the runner off 3rd by throwing to second to eliminate the run regardless if he crossed the plate. (The runner never stepped on third before advancing) But, the defense walked off the field thinking they recording 3 outs and the runner did not score.
4. The fourth out becomes the necessary doubling up of the lead runner.
