I’ll have to find it, but there is Yu Darvish reporting noticing Brewers players suddenly peering out toward their home bullpen then back toward the pitcher after the sign is put down. And then sure enough someone found footage of Yelich doing it.
Yelich even went on the offensive and tweeted something along the lines of “Hey man, nobody needs help hitting against you!”.

I spoke with someone on the coaching staff with the Twins. He said that when they played the Red Sox every time a breaking ball was the pitch called, there was a whistle from the Red Sox dugout.

Details from the Athletic story:
Three people who were with the Red Sox during their 108-win 2018 season told The Athletic that during that regular season, at least some players visited the video replay room during games to learn the sign sequence opponents were using. The replay room is just steps from the home dugout at Fenway Park, through the same doors that lead to the batting cage. Every team’s replay staff travels to road games, making the system viable in other parks as well.

Worth noting: Players are not being targeted for discipline. Front-office and on-field personnel, who were read into league-issued memos clearly stating they would be held accountable for tech-based cheating, are. That includes Red Sox manager Alex Cora:
